At the third flight of stairs, I kind of expected a zombie or some sort of Silent Hill creature to jump out or maybe an alien that you would have to defeat in first person shooter style before you were granted access to The Sanctum.
That random piece of pizza on the ground is like that health pack you get before facing the end boss.
